After F.I.F.A divides the countries into groups continent vise they then have to qualify , the first two from each group qualify for the world cup.
If you are from the Us and want to go to the Olympics in swimming, first you have to make a time standard to qualify for Olympic Trials. There are many events, but you can swim a maximum of six at Trials. The top two finishers for each event go to the Olympics. There are 14 individual events, and there are also 4 relays.
The qualifying process differs for each event for each country. As far as I know of, there are Olympic qualifying tournaments and trials that are held within each countries (i.e. the World Championship was held in Vancouver, Canada this June). Usually the top 3 in each event will advance to the Olympics, assuming that they meet the Olympic qualifying standards. PS. The top 8 nations can have up to 3 athletes in each event while other participating countries can only send 2 per event.

To swim in the olympics, you have to be selected for your country's olympic team, which will typically be selected in the year leading up to the competition. Selection usually occurs at a national qualifying event, which you must achieve a qualifying standard to enter.
