This would be impossible to determine since an ERA is determined by complete innings pitched. Since this pitcher did not get any outs, in theory the ERA would technically be considered INFINITE.

If a pitcher has men on base and has to leave game and another pither comes in and gives up a run when the game is tied who gets the loss if it happens in the 9th inning?

The pitcher that left the game with the game tied and runners on base would receive the loss if the relieving Pitcher gives up any of those runs because the previous Pitcher would be considered the current Pitcher of record until either the 9th inning ends or until all runners the previous Pitcher allowed to get on base either scores or are called out.

Pitcher leaves game relief pitcher enters with two runners on base and one run lead does relief pitcher get win or save?

It will depend on the circumstances. If this happens in the top of the ninth inning, he will get a save if he retires the remaining batters without giving up a run, but if he gives up one run to tie the game, so that the game is tied in the middle of the ninth, he will get a win if his team gets a run in the bottom of the ninth inning. How many visits can a pitcher get from a mananger in a game using the speedup rule?

In Major League Baseball there is no limit per game, but on the second visit in an inning, the pitcher must be replaced. Therefore, in a 9-inning game, a manager could visit the starting pitcher 9 times.
