Mask, fins, cylinder/tank, regulator (what you breath from) with an SPG (submersible pressure gauge ... so you know how much air/gas is left to breath) and with an alternate air source (an extra hose for your dive buddy in case it is needed), depth gauge, timer such as a watch if it is not part of your depth gauge, BCD (Buoyancy Compensator Device) which is the jacket you wear that you can inflate and deflate, a back plate (which holds the cylinder and which is often part of the BCD)
Optional (depending on type of dive and conditions):
wet suit or dry suit with boots, gloves and hood, weight belt with weights, knife or cutting tool, dive computer (or dive planning software), and a snorkel
