There are four teams from each league that make the playoffs. There are three divisions in each league and each division winner is guaranteed a place in the playoffs. The fourth team that makes the playoffs in each league is the team that has the best overall season record of those teams that did not win their division. This team is called the 'wildcard' team.

So, there are 16 regular season games and 5 weeks (4 rounds) of playoffs.

In total there are 16 teams that advance.
In most cases, the division winner with the best overall season record plays the wildcard team and the other two division winners play each other. But, in the case where the wildcard team and the division winner with the best overall record come from the same division, the division winner with the second overall best record plays the wildcard team and the division winner with the best overall record plays the division winner with the third best overall record. Confusing? Kinda, and kinda not really fair either, but MLB does not want two teams from the same division playing each other in the first round of the playoffs.
The first determining factor for the playoffs is whether you won your division. Division winners get automatic playoff berths, regardless of their record. Overall records only come into play for tiebreakers.
