Never, a player can not be called out since it is not baseball. But a player can be ejected from the game
I think you got baseball and basketball mixed up. Only baseball has umpires. In baskeball its a referee. Why there is a referee? Basketball needs a ref because he keeps the rules and watches out for fouls made from a player or team.
A good quality of a basketball referee is to read the play, if a player gets a small foul but does not fumble the ball a good referee will not call the foul. Another good quality a referee has to have is good communication skills, a referee should use his voice more than his wistle to keep the game flowing
They won't play the second game and will get a red card
If a player illegally kicks a ball the referee will call out "kick". that simple.
In basketball, if the player with the ball is moving (walking) without dribbling it is a violation called 'traveling'.

no because he might cheat for the person the ref is related on
The referee should reprimand them
Holding up one, two or three fingers are hand signals for various things (e.g. three seconds in the key) used by the referee. Because of this, players are barred from using those numbers to avoid confusion when the referee signals a player's number.
It all depends on what sport and how they were injured. If the player broke their leg by kicking a ball weird in soccer, it is the player's fault they were hurt. If a referee throws a basketball at a player's head in a basketball game, it would be considered the ref's fault if there are any damages.
