According to the International Ice Hockey Federation, the thickness needs to be about 3cm - see the link for further technical guidelines.

∙ 14y agoThe ice on a hockey rink is approximately 3/4" thick. The thicker it is, the slower is is to skate, and that is not good.

They are painted onto the floor. Then the floor is covered with ice. Because the lines are under the ice, instead of part of the ice, resurfacing the ice won't remove the lines.
