The batter would be out. However, if both feet were in the batter's box, then the batter would not be out and it would be ruled a foul ball.

I suppose if his toes were touching home plate but the rest of his feet were not, then that would not be "entirely" outside.

It is ruled a strikout if you ever bunt the ball foul with two strikes.
