Top tier players are essential to their team and will most likely be on the pitch for most of if not the entire game. There are no time outs or rest periods, except in modern American college soccer, which means that you could possibly be running for 45 minutes (each half) at a time up and down a field. Depending on which position you play the running varies(outside backs and midfielders typically do the most running for long periods) and if you aren't fit to play, a coach might not play you at all.
