I presume you mean a STARTING pitcher, listed as such on the initial lineup. When a reliever comes in, it's not unusual for the manager to remove a fielder and place the pitcher in the batting order such that he would be the ninth batter the next time his team comes to bat -- meaning the reliever might be anywhere from first to ninth.
Even during the regular season, and even when a pitcher is a very good hitter (Don Newcombe hit .359 in 1955, and was used as a pinch hitter 23 times), placing the starting pitcher anywhere but ninth in the initial batting order is EXTREMELY rare (other than Tony LaRussa). The only recorded time this happened in a World Series was Game 4 of the 1918 Series. No surprise -- Babe Ruth was the pitcher, and he batted sixth.
SS Roy Hughes, a single, in the 9th inning of Game 7 of the 1945 World Series.
The American League began using the DH in the 1973 season. The last AL pitcher to bat in the 1972 season was Rollie Fingers of the Oakland Athletics in Game 7 of the World Series. He grounded out to 3rd base in the 9th inning.
In Game 4 of the 1918 World Series between the Red Sox and Cubs, starting pitcher Babe Ruth batted 6th in the lineup. Catcher Sam Agnew batted 9th.

Mike Shannon. Shannon, of the 1968 St. Louis Cardinals, hit a home run in the bottom of the 9th inning of Game 7 off of Mickey Lolich of the Detroit Tigers who had hit a home run in the 3rd inning of Game 2 of the '68 Series, his first ever WS at bat.
No, he has not. Derek Jeter has been a member of two Yankees teams that lost the World Series. In 2001, the Arizona Diamondbacks won game 7 on a walk-off single in the 9th inning. The second loss came in 2003 to the Florida Marlins, where Jorge Posada hit a ground ball to the pitcher for the final out in game 6.
Jeff Weaver of the St. Louis Cardinals.Weaver was the starter and went 8 innings giving up 2 runs on 4 hits. Adam Wainwright came on in the 9th to get the save in the Cardinals 4-2 victory which clinched the World Series for them.
