Umpires are the officials in a softball game. The plate umpire is the head umpire for that game. A coach can approach the umpires about any rule interpretation questions, but judgment calls are final and cannot be reversed.
The role of a captain is to make sure that everyone is fit and healthy for the game and is all prepared. The captain is to do the toss with the opposition captain, report the result back to the umpires, go to umpires with querys regarding calls etc.
I would have a system similar to cricket, or rugby league or other sports with qualified umpires working as video umpires or refs. Umpires could call for assistance on close calls. You could also have teams being allowed a certain number of challenges at the discretion at the coach.

Umpires do not have the ability to see everything from every angle. Sometimes they only catch the play from a glance. Sometimes it just looks different from the angle they are standing versus the angle of the bench or fans. And sometimes it is just a simple mistake in judgement.
The Umpires get together and the Umpire can call interference on himself and the playing running or batting has to redo it
