I believe that is Nottingham Forest - they won the league in 1978, their only domestic success, the went on to win the European Cup in 1979 and 1980. It would be easier for a team to join them in this category nowadays that the teams placed 2nd -4th in some leagues can join Brian Clough was a legend

Notts Forests won the European Cup in 1979 and 1980 and subsequentley were relegated to the 2nd Division in 1992-93. As far as I know they are the only Team in European to have won a European Cup and been relegated to the Third Teir of English Football in 2004-2005
