It has only happened once, in the sixth game of the 1967 World Series at Fenway Park between the Red Sox and Cardinals. Carl Yastrzemski led off the fourth inning with a home run and after two were out, Reggie Smith and Rico Petrocelli hit back to back shots. The Red Sox won the game 8-4 forcing a game seven which the Cardinals won 7-2. All the homers came off Cardinal pitcher Dick Hughes.
