MLB conducts a coin toss prior to the conclusion of the season. All teams who have the possibility of being the wild card (or the possibility of a tie for division winner) are included and all possible scenarios are covered. The team winning the coin toss hosts a one-game playoff to see which team advances as the wild card, or which team wins the division, if that is the case. If two teams are tied for thhe division lead, and also tied with a team from another division for the wild card, the two teams tied for the division lead will have their tie-breaker game played first. The winner of that game is the division champ, and the loser of that game plays a one game tie-breaker against the team from the other division to determine the wild card.

If teams from the same division finish the regular season in a tie, they play a one game playoff to determine the division championship. The exception is if the other team would make the postseason as a wild card. In that case the team with the better head to head record wins the division and the other team takes the wild card.

At the end of the season, there may be a tie with the wildcard teams or even in a division itself. In the case of a tie, those teams must play one game to determine who will finish the regular season with the best record.
If these teams are not going for the division or wild card, there will be no tiebreaker.
Otherwise, here is how it would work.
First, two of the teams would be decided to play against each other. The winner of that team would play against the third team. The winner of that game would move on.
If the winning team is the division titlist and the two losing teams are tied for the wild card, the two wild card teams would play one more tiebreaker game to decide the wild card winner.
In the event of 2 teams being tied for the Divisional lead and also being tied with another team that is in the Wild Card standings then the 2 teams that are in the divisional lead race will play a tie-breaking game to determine the winner of the division and the team that wins the tiebreaker will win the Division while the other 2 teams wild be the two Wild Card teams and will be responsible for playing against one another in a Wild Card game.
