To my knowledge, Tim Wakefield of the Boston Red Sox, before he became a Red Sox player, threw 215 pitches lasting just under 15 innings. Because he throws the Knuckleball, he requires little effort to throw.

The most pitchers used in one major league game was a combination of the two teams of the St. Louis Cardinals and the Pittsburgh Pirates on September 30th 2007. The St. Louis Cardinals also set a major league record that day using 10 pitchers in one game while the Pirates used 8. -Tibbs
I don't know that pitch counts were kept way back when but the highest I have heard of is when Tom Cheney pitched a 16 inning complete game for the Washington Senators against the Baltimore Orioles on September 12, 1962. He threw 228 pitches and struckout 21 batters. The 21 strikeouts is an MLB all time record for strikeouts in one game.
In 1974 Nolan Ryan threw a 12 inning 254 pitch, 11 strikeout game.
On many occasions, a relief pitcher has come into the game, then been substituted without ever throwing even one pitch, so the answer to the question as worded is 0.
