I know for a fact that the Los Angeles Dodgers have done it. They did it in the 9th inning on Monday September 18th 2006 while playing the San Diego Padres. Jeff Kent and J.D. Drew opened the ninth with homers off San Diego's Jon Adkins. Russell Martin and Marlon Anderson then both knocked it out the park on the first two pitches thrown by Trevor Hoffman. The Minnesota Twins also Did it on May 2nd 1964 in the 11th inning to defeat Kansas City.
