Yes and No, Goalies in the NHL do NOT have to wear one As the "NHL Rules" only said that "Goaltenders are highly suggested to wear a sort of protective helmet" which means they don't have to if they don't want to. But if they come into the game with one they must wear it during the game but if they show up without one no one can stop him from playing because of it. But players do.

Batters wear helmets, catchers wear face masks.
The technology at the time would have been relatively primitive, and masks ranged from chemical-impregnated pouches attached to the full-face precursors to modern gas masks. For example, the french 'Tampon T' is a multi-layered fabric pouch with a face strap. It was usually issued with goggles to protect the wearers eyes. Full face gas masks tended to have asbestos-based filters and worked relatively well. Of course these days the asbestos will have degraded and it is VERY dangerous to wear such a mask unless the filter has been drilled out.
The Masks of Death was created in 1984.
Egyptian masks were an important part of burial practices during ancient Egyptian times. These masks were often buried with corpses, and the Egyptian people adorned the corpses of royalty with masks. The masks were meant to provide a face for the dead spirit as it went through its travels. They helped to protect the "Ka," or spirit.
