Without on board oxygen, the limit is about 15,000 feet. With on board oxygen, jumpers can go to about 20,000 feet. With bail out oxygen bottles, the altitude is limited by the type of aircraft used. The record for highest parachute jump is just shy of 103,000 feet. http://en.wikipedia.org/wiki/Joseph_Kittinger
That depends on what type of parachute jump you are doing. If you are doing a demonstration jump into a football stadium, it can be as low as 1600ft. If you are doing a formation skydive, it can be as high as 15,000ft (without oxygen). During a formation skydive, a group of jumpers will freefall together with packed parachutes, only opening them later after a minute at a height of around 3,000ft.

From the altitude of the airplane when you jumped out.
Indoor skydiving is proven safer then actually jumping out of an airplane at 3,000 feet. It is significantly cheaper than real skydiving and it is safer without the need for a parachute or a plane.

You can take part in a high sky dive in tandem from the Skydiving website. Tandem skydiving is the safest way to experience skydiving and typically costs between $150-$350.
The risk of a heart attack during a skydiving is the same as it would be during any other high adrenaline experience. If considering skydiving with a heart condition that could lead to a heart attack, consult your doctor to make sure you are physically fit for skydiving and inform the instructors at the drop zone you are going to.
There is no law against a minor skydiving, but very few "drop zone" owners will take the risk - their liability insurance would be endangered.
You have to be 18 at the majority of dropzones. A select few will drop that down to 16 with parental consent.
There are only about a dozen to two dozen skydiving fatalities annually. Although any fatalities are tragic, given the number of people that skydive at the hundreds of drop zones around the world, this number is very reasonable.
Skydivers in different freefall orientations fall at different speeds:Wingsuiting - 25 to 60mphBelly to earth - typical for formation skydiving - 120mphFreefly headup and headdown - 150 to 190mphSpeed skydiving - 200 to 327mphFor each of these disciplines, experience, jumpsuit worn, weight & height all make subtle differences to exact freefall speeds. During any typical skydive, speeds vary a lot.
Yes, being an extreme sport skydiving causes high level of emotions. It's possible to cause headaches or other health problems to people who aren't in good health. However headaches are in no way a typical problem caused by skydiving.
If first you don't succeed, skydiving is not for you.
