They do in little league, but the action of pitching a softball is less harmful to the body than an overhand pitch, asa and usssa haven't seen the need for it as yet, there is not enough data on injuries due to extended pitching underhand.

To be a decent softball pitcher it could take 3-4 years to pitch a softball. Fastpitch that is. The ball is bigger and weighs more than a baseball. The wind up is tricky. Most pitchers that I know have to practice their pitching around 3 to 4 days a week.

Travel will compete in tournaments which will only last 1 to 2 days, but have up to 8 games in a day! 8 being probably the most. Back in the 60s, it was raised (the mound was made higher) since the pitchers complained about the batter's advantage.
