Frank Chance was the player/manager of the Cubs that defeated the Tigers in both the 1907 and 1908 World Series ... he also played first base. Chance was player/manager of the Cubs between 1905-1912 and also led the team to the 1906 and 1912 World Series that they lost (1906 to the White Sox and 1912 to the Red Sox). He was inducted into the Hall of Fame in 1946.

This is more of an opinion question. I would say that Joe Torre was most successful as a manager, because he led them to the postseason for 12 straight years, winning 4 World Series and 6 AL Pennants. Managers Joe McCarthy and Casey Stengel have each won 7 World Series for the Yankees.
They were in the world series in 1924, 1925 and 1933 only winning in 1924.
If you mean the 1912 World Series, that was the Boston Red Sox.
The San Diego Padres appeared in the 1984 and 1998 World Series.
