It has only happened once, in the sixth game of the 1967 World Series at Fenway Park between the Red Sox and Cardinals. Carl Yastrzemski led off the fourth inning with a home run and after two were out, Reggie Smith and Rico Petrocelli hit back to back shots. The Red Sox won the game 8-4 forcing a game seven which the Cardinals won 7-2. All the homers came off Cardinal pitcher Dick Hughes.

Has there every been a world series game that went to the ninth inning with a 0-0 tie?

Game 2 of the 1914 World Series. Boston Braves scored a run in the 9th to beat the Philadelphia Athletics. Game 1 of the 1949 World Series. Yankees scored in the bottom of the 9th to beat the Dodgers. Game 6 of the 1956 World Series. Dodgers scored in the 10th to beat the Yankees. Unbelievably, Game 7 of the 1991 World Series was tied 0-0 going into the 10th inning. Minnesota won the game (and the series) over Atlanta in that inning. There may be others, but I don't have the time or energy to check every game of every series.

Through the 2008 World Series ... In a single World Series the record is 27 by Christy Mathewson of the New York Giants in 1905. He threw three complete game shutouts in helping the Giants defeat the Philadelphia Athletics, 4 games to 1. Over more than one World Series the record is 33 2/3 innings by Whitey Ford of the New York Yankees. The streak started in the first inning of Game 1 of the 1960 World Series ... Ford threw shutouts Game 1 and Game 5. It continued in the 1961 World Series as he threw a shutout in Game 1 and 5 scoreless innings in Game 4 to extend the streak to 32 innings. It ended in Game 1 of the 1962 Series with two outs in the second inning.
