Well, there was Pete "Pistol" Maravich taht actually had " 'Pistol' " written on his jersey, with quotes and everything.

Then there's also Nenê Hilário, from the Washington Wizards, who doesn't have his original last name in there too. Actually, in his case, Nenê wasn't even his name, it was a nickname to his name that was Maybyner Rodney Hilário, but was changed to Maybyner Rodney Hilário Nenê due to the fact that, in Brazil, it's very usual to call athletes by their first names or nicknames, and that includes the name on their jerseys. It's rare to see a Brazilian athlete with their last name on the jersey, much like soccer players Ronaldinho, or Robinho; but different from Argentinian soccer player Messi, whose first name is Lionel. So, thanks to the popularity of his nickname, Nenê uses it on his jersey, instead of Hilário.

If there's anyone else that used anything besides their last name on their jersey in the history of the NBA, or ABA, I'm not aware of.
