Adam Vinitieri
Scott Norwood
Garo Yepremian
Rich Karlis
Of the four kickers listed above, the two that stand out to me are Yepremian and Norwood. Norwood missed a 47 yard field goal with eight seconds left in Super Bowl XXV that gave the New York Giants a 20-19 victory over the Buffalo Bills. Garo Yepremian may be responsible for the biggest blooper in Super Bowl history. With less than 3 minutes left in the fourth quarter of Super Bowl VII and the Miami Dolphins comfortably ahead of the Washington Redskins 14-0, Yepremian was sent out to attempt a 42 yard field goal to ice the game. The kick was blocked and the ball came straight back to Yepremian who, instead of simply falling on the ball, decided to attempt a pass. The ball slipped out of his hands, he then batted it up in the air, and it wound up in the hands of Redskin CB Mike Bass who returned it for a TD. Miami was able to hold on to a 14-7 victory to become the only team in NFL history to go undefeated throughout the regular and post season. But the Yepremian play will live on for generations.

Through Super Bowl XLIII, that is Don Chandler of the Green Bay Packers against the Oakland Raiders in Super Bowl II and Ray Wersching of the San Francisco 49ers against the Cincinnati Bengals in Super Bowl XVI. Each made 4 field goals.
If you mean the game was won by kicking a winning field goal, there are 3 I know of.
1. In the 1991 superbowl, the Giants beat the Bills 20 - 19 by kicking a late field goal.
2. In 2002 The patriots won with a fieldgoal kicked by Adam Vinatieri.
3. In 2004 The patriots again won with a FG by Vinatieri.
Also Super Bowl V when Jim O'Brien of the Baltimore Colts kicked a 32 yard field goal with 5 seconds left to give the Colts a 16-13 victory over the Dallas Cowboys.
The record is for missed field goals in a Super Bowl is 3 by Efren Herrera of the Cowboys in Super Bowl XII.
