In Major League Baseball, this is covered under rule 6.05(l).

1. With a runner on first base (whether there is an additional runner or runners on any other base does not matter), or with first base open but runners on both second and third, and less than 2 out, the batter is out and the ball is dead. All runners return to their previously occupied bases.

2. With 2 out, or a single runner on only second or third base, the play remains alive.
