There is no rule in MLB that specifies how many times a catcher may visit the pitcher during a game.
There are very few sidearm submarine pitchers in the MLB but to name a few there is Mike Myers, Cla Meredith, Chad Bradford, and Mike Koplove.

In a regular MLB game, it is about 60 feet. Normally it should be the same distance from the pitcher's mound to home plate.
