Yes, it only happened once... It was done by Carroll Hardy, Hardy pinch hit for Williams on September 20, 1960, when Williams fouled a ball off his right foot and could not continue the at bat.

Williams fouled a pitch off his foot in the first inning of a game against the Baltimore Orioles on September 20, 1960. He could not continue the at bat and Hardy was summoned to replace him. Hardy hit a line drive to the pitcher that was caught and turned into a double play when the pitcher threw to first base.

Hardy replaced Williams in left field and went 1-4 in the Orioles 4-3 victory.
