Depends on the relative strength of the national league, which is determined by the UEFA Coefficient. See more at the related link. Basically, weaker leagues have fewer representatives and start earlier in the competition. Few teams make it from the first couple of rounds into the group stages; a notable exception was APOEL Nicosia from Cyprus, who began in the second qualifying round and made it all the way to the Quarter-Finals, their run eventually ended by Real Madrid.
