Usually no. Typically there is a built in bra and a liner. Wearing a bra and underwear usually gets bulky especially when you are moving around a lot.

If it is a competition and the gymnast needs underwear, there are special skin colored ones because any underwear showing can be cause for a .10 deduction (same goes for bra straps showing). If the gymnast is comfortable, underwear is really not necessary until puberty hits.
