The distance a horse can maintain a gallop depends on their build and physical fitness. A well conditioned horse can easily maintain a gallop for a mile to a mile and a half. At two to two and a half miles most horses will feel fatigued.

This depends upon the breed of the horse, the weight of the load carried/pulled by the horse, the condition the horse is in and the terrain the horse is running over. In general, a warm-blooded horse in good condition with no rider can gallop across even terrain for a couple of miles; the same horse under the same conditions can canter for 4-5 miles.

However, at the end of this, the horse is likely to be very tired and unable to run much anymore.

It depends on the horse. Some horses are more sensitive to commands to 'go', while others would much rather take it slowly. Faster horses include the thoroughbred and the quarter horse, and slower horses may be ponies and draft horses. Lean, young horses aged 2-16 will have the fastest reaction time if they go on command and are not lazy. That is why racehorses are so young and usually retire at about age 5 or 6.

The trot is the horses main travelling gait (walking is for grazing, and galloping is for flight/ play.) A horse can travel anywhere from 10 to 100 miles in a day depending on the terrain, weather, and speed of the gait as it will vary a bit over different terrains.
At 5 miles per hour a horse could likely travel in a competitive state with a rider for almost 100 miles as it will not be looking for food or water. In a non-competition situation and depending on if the horse is feral/ wild or domesticated then it'll travel likely between 10 and 30 miles due to having to forage for food and search for water.
