It was Pinehurst Resort and Country Club (Answer D). Go to Pinehurst's web site and look under the 1930's It was called maniac hill.

The first independent driving range in the US was located in Flourtown, PA. It was a stand alone facility owned by Robert Seka. He opened it in the 1950's. It is no longer there.
