It is basically their course guide, they have planned what shot is best for each hole, what club they normally hit, actual yardage, adjusted yardage (taking wind, heat and slope into consideration), they have the greens mapped out - where to hit your approach and how the green breaks. It is basically their Bible.

Depends. They would usually play in a pro-am event, which would cost the amateur golfers a lot of money, but the money wouldn't necessarily go to the player. If it was just a casual round it is unlikely that they would charge a fee, but they may do.
