Generally your first step is the pushaway. You want to drop the ball into the swing and start moving your foot at the same time. The precise moment your foot hits the ground your elbow should be locked.

Your last step is USUALLY a slide, however some people do take a real step (called planting) to gain leverage. You release the ball at the bottom of the swing as you take this step.
You should maintain fluidity throughout the entire approach and keep your arm swing as loose as possible.
