Use a Stanley knife with a hooked blade to remove the old grip. Peel off the old tape, and use grip solvent or white spirit to remove the residue. Place the shaft in a rubber vice, as if you were at address. Then get a piece of grip tape and place it on the end of the club, leaving enough for an overlap which should be folded and put inside the shaft. Get your grip, pour or spray some grip solvent or white spirtit into the grip, whilst holding the hole at the bottom, then shake, and through the hole in the bottom allow the liquid to pour over the tape, so it is well covered and even add more if necessary. Finally, and quickly align the grip and slide it over the tape, take the club out of the vice, and place the club at address to make sure you are happy with it. Then leave to dry, for at least a few hours.
The best thing to do is to take it to your local Golf shop/ professional or clubfitter.
To DIY it, you will need to buy a shaft cutter for steel shafts, or use a circular rotating blade for graphite shaft. Simply have the grip removed and then then mark how much you want to take off, then cut it, you may need to shave it down slightly. Then regrip the club.
