Typically, you take 50% of the combined handicaps of both players. Then you apply the handicap to the course handicap holes.....So if the players have a total 14 handistrokes they would get a stroke in Handicap holes 1-14....

One way is just like in a stroke play tournament.Lets say your foursome has a 5, 7, 10, 11 handicappers. Then you would adjust the 7 by giving 1 stroke on the 2 hardest holes, 5 hardest holes for the 10 and so forth. However, you must all tee from the same tee box.
Probably cause its really hot there and they have deserts there also. I am not sure though.
The best way is to calculate the number of degrees of separation you wish to have between the holes. The formulas can be found in any geometry text. One can then mark the spots with a pen or marker before making the actual holes.
