Through Super Bowl XLIII, that has happened twice:
1) Don McCafferty, Baltimore Colts - won Super Bowl V in his first year as head coach.
2) George Seifert, San Francisco 49ers - won Super Bowl XXIV in his first year as head coach.

The first two "Super Bowl"s weren't titled as such, but named so retroactively. Super Bowl III, which saw the New York Jets defeat the Baltimore Colts was the first titled Super bowl, so the first coach to lose would be Don Shula.
Tom Flores won a Super Bowl as backup quarterback for the Chiefs in Super Bowl IV and as head coach of the Raiders in Super Bowl XV.
