June 10, 1960, Cincinnati Reds catcher Dutch Dotterer hit a grand slam home run off Koufax in the top of the 2nd inning, at the LA Coliseum, scoring Lee Walls, Willie Jones and Gus Bell, to give the Reds a 4-0 lead. After walking the next batter, opposing pitcher Bob Purkey, Ed Palmquist replaced Koufax. The Dodgers lost 4-3, Purkey was credited with the win, to raise his record to 5-2, while Koufax's W-L record fell to 1-8. Koufax finished the 1960 year with an 8-13 record (his last sub .500 pitching season).
Unfortunately, the most recent data I could find was for the 2008 MLB season. In 2008, 124 grand slams were hit (72 in the AL and 52 in the NL) in 2428 total games for an average of 1 grand slam every 19.58 games. The most grand slams hit in an MLB season is 176 (89 in the AL and 87 in the NL) in 2429 games in the 2000 season for an average of 1 grand slam every 13.80 games.
