I know that Deion Sanders wore this number in the NFL- I'm not sure if it was the same way with MLB. An easy way to look is to go to foxsports.com and go to the rosters of each team and scan down the column of numbers to see what active players wear it. In other sports, 21 includes LaDainian Tomlinson, Frank Gore, DeAngelo Hall,
I looked and so far I've found Ryan Klesko of the Giants, Heath Bell of the Padres, Mark Sweeney of the Dodgers, Clint Barmes of the Rockies, Kip
Wells of the Cardinals, Ty Wigginton of the Astros, Scott Hatteberg of the Reds, Jason Marquis of the Cubs, Dmitri Young of the Nationals, Jon Lieber of the Phillies, Carlos Delgado of the Mets, Warren Spahn of the Braves (retired; thus no current Braves are wearing it), Sammy Sosa of the Rangers, Mark Kotsay of the A's, Matt Garza of the Twins, Billy Butler of the Royals, Sean Casey of the Tigers, and, lastly, Nick Markakis of the Orioles.
go to wikipedia and look at '21' and the sports sub-section in it. it is pretty comprehensive.
Paul O'Neill of the New York Yankees wore the number 21
Updated answer: Here are players that have had their #21 retired by their respected teams. Bob Lemon (Indians), Warren Spahn (Braves) Roberto Clemente (Pirates)
No one. Warren Spahn wore #21 as a member of the Boston/Milwaukee Braves and that number has since been retired as have the #s 3 (Dale Murphy), 35 (Phil Niekro), 41 (Ed Mathews), 44 (Hank Aaron), and the 42 of Jackie Robinson that has been retired by MLB.

The numbers have colors designed to reflect the uniforms worn by players of different eras. As a result, the red 21 representing Warren Spahn looks like something from the old Boston Braves uniforms. Meanwhile, the blue 3 worn by Dale Murphy produces memories of the uniforms the Braves wore in the 1980s.
