Ray Chapman, the shortstop for the Cleveland Indians, was hit in the head and killed by a pitch thrown by Carl Mays in 1920. Chapman was one of the leading shortstops in the league at the time. Joe Sewell replaced him, and went on to a Hall of Fame career. The Indians, led by Tris Speaker, won the pennant and the World Series.

None have died on the field, but in 1920 Indians shortstop Ray Chapman was hit in the head by a pitch (they didn't wear helmets back then) and died the next day.
