As of September 1, 2007 this has never happened. There are 12 players that have hit two grand slams in one game and only one player, Fernando Tatis, that has hit two grand slams in the same inning. Click on the 'Two Grand Slams in a Game' link to see who the twelve players are that have hit two grand slams in one game.
Many active players have accomplished the feat, including Josh Hamilton (the last player to hit four homers in a game), Albert Pujols, Alex Rodriguez, Jose Bautista, Aubrey Huff, Carlos Beltran, Jason Giambi, Corey Hart, Casey McGehee, Chris Heisey, Curtis Granderson, Carlos Gonzalez and Shin-Soo Choo.
Pujols, now with the Los Angeles Angels of Anaheim, has had five career three-homer games, including one for the St. Louis Cardinals in Game 3 of the 2011 World Series.
Many MLB players have hit 3 home runs in a single game. My personal favorites to do it are Mike Lowell and Cody Ross, who accomplished this feat as a members of the Florida Marlins.
Once in MLB ... Jim Tobin of the Boston Braves did it on May 13, 1942 against the Chicago Cubs. He pitched a complete game that day and won 6-5.
Jim Northrup of the Detroit Tigers hit a record three grand slam home runs in one week in June of 1968.
Jim Tobin on May 13th, 1942.
