Jim Plunkett, Oakland Raiders and Stanford
The last Heisman winning QB that started and won a Super Bowl was Jim Plunkett who won the Heisman in 1970 (Stanford) and won Super Bowls XV and XVIII with the Raiders.
The only other Heisman winning QB to start and win a Super Bowl was Roger Staubach who won the Heisman in 1963 (Navy) and won Super Bowls VI and XII with the Cowboys.

And it could go to 19 if Richard Dent of the Bears (Super Bowl XX MVP) is elected.

However, Baylor has won both in the same academic year, with Robert Griffin III winning the 2011 Heisman Trophy (presented in December 2011) and Brittney Griner winning the 2012 Women's Naismith Award (presented in April 2012).
Paul Hornung was the 1956 Heisman Trophy winner who played on a Notre Dame team that finished the season 2-8. He is the only player from a losing team to win the Heisman Trophy.
Reginald Bush II won the 2005 Heisman Trophy, but due to theft, he got it taken away. Some say, the runner-up, Vince Young, should take it, and some say that the University of Southern California should keep their trophy, and that Reggie's heisman should just be taken away.
