I found two ... I am sure there are many more.

Weightlifter Howe Liang "Tiger" Tan competed for Singapore at the 1956 and 1960 Games and for Malaysia at the 1964 Games.

Swimmer Sarah Poewe competed for South Africa at the 2000 Games and for Germany at the 2004 and 2008 Games.

Luis Monti played for Argentina in 1930 and Italy in 1934, being runner-up in the first and champion in the later.

Ferenk Puskas played the 1954 FIFA World Cup for his native country Hungary, then became a spanish citizen and played for La Furia in 1962

Jose Santamaria played for Uruguay in 1954 and Spain in 1962

Jose Altafini, at the time known in Brazil as Mazzola, won the 1958 FIFA World Cup with Brazil and played for Italy in 1962.

There are also some recent cases of former yugoslav players: Robert Prosinecki, who played the 1990 World Cup for Yugoslavia and the 1998 and 2002 tournaments for Croatia. Prosinecki is the only one who scored for two countries.

And Dejan Stankovic, who played for Yugoslavia in 1998, Serbia and Montenegro in 2006 and Serbia in 2010.

Some other players were called up by two different countries, but played only for one or even none of them.
