Neil Sasche accidental incident with a Fitzroy player. Left him a quadriplegic - though later he did manage to recover some arm movement. He now has a foundation researching spinal chord trauma

The John Greening incident was a terrible Deliberate incident behind play - put him in a coma for 2 weeks and took him a year or two on rehabilitation to recover sufficiently to play again!

Both of these incidents occurred during the 1970's.

There have also been recordings of deaths in some football leagues.
