Yes, getting hit with a paintball can be painful and even leave bruises. Be sure to always wear proper safety gear to reduce the potential pain. Of course, if you don't get shot you won't get hurt.
Assuming you play at a regulated field with experienced Referees, most fields require that each marker gets "Chronographed" or Chrono'd before play. The chrono will tell the referee how fast the gun is shooting in "Feet per Second"s and will then make adjustments to the marker. Most fields only allow 280fps or lower. This will insure that everyones markers are shooting at a competitive and yet safe limit. A Chrono'd gun will feel like a small pinch when you get shot as opposed to the ridiculous and scary results you see on YouTube from kids playing in their backyards with no chrono available. I used to tell first timers that it only hurts enough to remind you to duck next time!

A paintball chronographed at 290 FPS from about thirty feet will hurt about as much as a thick rubber band snap on the neck, except for anywhere you get hit. You can reduce the pain buying a chest and back protector, wearing extra layers of clothes and gloves and establishing a minimum shooting distance.
