Theoretically this makes sense, because increasing the intraabdominal pressure (this is called the Valsalva maneuver) during exercise will force blood out of the abdomen and into the lower pressure area of the testicles, causing a swelling of the venous system. Theoretically, if tight underwear is worn that increases pressure in the veins of the testicle, the swelling of the varicocele may be reduced.
