I assume you are the student. Hope this gets to you in time. You need a physical and your parents to sign the consent form. Also, a pair of cleats and socks, the rest should be provided for you.

No, as it's not compulsory to do it, on the other hand if football is part of that lesson you are in you do have to take part, in that class :/
