A lot, You can go to the usfa web site and download the rule book. There are rules on each weapon. There are rules on gear, and rules on the bouts. and pretty much everything that came up in a match over 100 years, and they are still making rules almost every year. It a sport that changes over time, and we do go back to old rules sometimes. So you have to get checking the rules.

Is there or are there specific patterns to fencing with swords or is it all arbitrarily made up by the two opponents?

Foil and Saber both have rules of right of way which makes patterns fairly common. Epee does not have right of way rules and thus has fewer predictable patterns. However there are only so many reasonable ways to attack and defend, therefore there are patterns in fencing but it is simply because of convenience not because it is required.
