The 10th degree black belt would be awarded by the governing body of the organization and the existing 10th Dan. In most cases this requires you to be at least 60 years old and to have been with the organization for many decades. If you encounter those that are under the age of 50 claiming to be a 10th degree black belt, don't believe them! It is pretty rare to have a 5th Degree under the age of 30, the youngest in my organization would be 28 for that rank.

Black belt is the highest. Though traditional Okinawa karate uses a red and white belt to indicate 7th and 8th degree black belt and solid red for 9th and 10th. Many American schools use the red belt as a kyu level below black belt.

Most schools, or types, of Karate use the Kyu/Dan levels that started with Judo. The colors of the belts indicate what level the student is at. Most schools use the following colors - White, Yellow, Red, Green, Blue, Brown, Black. Once you reach Black, you then move to the Dan system. It then becomes Black Belt Dan 1st, Black Belt Dan 2nd, Black Belt Dan 3rd, up to Black Belt Dan 10th. So, to answer your question, technically the highest level belt would be Black Belt Dan 10th. In reality, each level that you attain is the highest because you have surpassed your previous best.
Most black belts are made of silk. They last a lot longer then the cotton belts normally used for kyu levels and you will have the black belt for the rest of your life.
It depends, for the Senior fighters, the second highest colour belt is brown, but the second highest belt is actually your 9th black belt (9th dan, this is pretty much knowing every throw, and only one person has ever reached it) Juniors, the second highest belt is blue, after brown, as you cannot be a black belt junior. :D
