In order to set up an authenticator with a battle.net account, you first log in to your account. You will find under your World of Warcraft game the option to add an authenticator. It will ask you to fill in the serial number found on the back of the authenticator. Once this is done, the process is complete.
If you have lost your authenticator, you will have to call Blizzard Billing during office hours. They are the only ones that can remove an authenticator if you have lost it. You can also try contacting a CM on the official wow forums in the customer support area, but you still will probably have to call Blizzard Billing to complete it.
To get the physical authenticator, no, you have to pay for it, which basically just covers the shipping and handling. There are blizzard approved apps for a lot of cell phones that will act like an authenticator though. Check out the link in the related links to see a list of appoved apps.
This depends whether someone with a WoW account that does have an authenticator attached has logged into your game. If so, simply log in normally but leave the authenticator field empty. It will then log in normally. If not, or the above does not help, contact Blizzard customer support. There is a chance someone got hold of your account details, added the game to their account and/or attached an authenticator. When this happens, you can no longer log in to the game or your account.
