You need a physical to play sports for a couple of reasons. The first being if u had testicular cancer and it went unnoticed it could further worsen your condition. The second reason is that if u have HIV/AIDS your athletic coach needs to know so that if u start bleeding they immediately can take care of the situation so u don't spread it.
