I would say it was on November 4, 2001. It was game 7 of the World Series against the Arizona Diamondbacks. Mariano came in to the game in the 8th inning with a 2-1 lead and struck out the side, but in the 9th, the Diamondbacks would tie the score and eventually win the game and the World Series.
As of July 1st in the 2009 season Mariano Rivera has one blown save.

The Boston Red Sox.
Mariano has blown 52 saves in his career. Only 1 of those blown saves were against the Diamondbacks. The Diamondbacks have played the Yankees 19 times (including postseason) and have gone 7-12 against them. The single game Mariano blew was Game 7 of the 2001 World Series that gave the Diamondbacks the World Series
I would pick Miriano Rivera. He's always been pretty consistent and just because he's blown two saves doesn't make him a bad closer.
He has a total of 59 Blown Saves in his regular season career. Since 1998, he has 46 Blown Saves: 1995 - 1 1996 - 3 1997 - 9 1998 - 5 1999 - 4 2000 - 5 2001 - 7 2002 - 4 2003 - 6 2004 - 4 2005 - 4 2006 - 3 2007 - 4 2008 - 1 2009 - 1 (currently) TOTAL - 61(currently)
In the 2010 session Moreno Rivera blow two (2) save to the Rangers in the same year ... This was that First time that Moreno Rivera has blown two (2) saves to the same team in the same session ...
