The one on the left side is a tribal Y and M kind of like the Tiger Woods symbol but it means Yadier Molina. The one on the right side are musical notes. The notes seem to be, in order from left to right: D5, D5, D4, E4, F4. The furthest to the left are harder to read because they are smaller, so I may be wrong on what they actually are. When I played them, they sounded like nothing... just random notes. It's rumored to have something to do with Reggatone.

Short answer, yes, getting a needle covered with shoved into the skin of your neck thousands of time, a minute hurts. Slightly longer and more informative answer, probably not as much as you think. At least not if it is comparable to the tattoos i have, The back of the neck is mostly skin over muscle, and the nerve density doesn't seem too high, it should be comparable to back tattoos or my scalp tattoos.
